Breast Augmentation

One of the most commonly performed aesthetic procedures (> 250k / year)

Implant based augmentation can improve breast shape and size, enhance cleavage and provide lift to the breast

Beyond aesthetics: implants are used in breast reconstruction secondary to cancer, trauma or inherent breast deformities

Breast implants come in a variety of shapes, textures and sizes

The two main types of breast implants are saline and silicone

Silicone vs Saline

Silicone implants are often preferred due to a more natural feel 

Silicone implants are less associated with a rippling deformity compared to saline implants 

Saline implants provide early detection of rupture due to immediate deflation of the breast

Saline implants do not require surveillance imaging for rupture

With newer generations of cohesive gel silicone implants, rupture may be difficult to perceive

The FDA currently recommends an initial MRI or ultrasound 5-6 years after implantation of silicone breast implants and every 2 years thereafter

Implant Selection and Positioning

Choosing the right implant for you requires a careful balance between your goals of augmentation and your breast anatomy

Important measurements to consider when selecting the right implant include base diameter of the breast, implant volume and implant projection

Most common approaches to implant placement include inframammary fold or an incision around the areola

The breast pocket can be created under the gland of the breast, under the pectoralis muscle fascia or under the pectoralis muscle

While in consultation for breast augmentation, be sure to discuss with your surgeon the planned incision placement and implant pocket selection

New FDA Safety Recommendations

The surgeon must provide a patient safety checklist to ensure discussion of the following:

  • Situations in which the device should not be used or implant
  • Considerations for a successful breast implant candidate
  • Risks of undergoing breast implant surgery
  • Importance of physician education, training and experience
  • Risk of a rare type of lymphoma: BIA-ALCL
  • Risk of systemic symptoms
  • Discussion of options other than breast implants, as appropriate
